IF YOU WISH AN OCCUPATIONAL OR PHYSICAL THERAPY CONSULT ON THE SAME DAY AS YOUR MD APPOINTMENT, try set your appointment on a Thursday. An OT is often available both morning and afternoon and a PT is often available in the afternoon on Thursdays.

CANCELLATIONS

Since the wait for an MDA/ALS Clinic follow-up appointment can be as long as 2 months, if it is necessary to cancel an appointment, please do so well in advance so that the appointment can be offered to another client who may be in desperate need of a clinic visit.

MEDICAL RECORDS

REMINDER: Request any previous medical records and actual copies of MRI, CT scan, etc. prior to your appointment so that you can bring them with you on your first appointment. You may also want to have name, address and phone of your current neurologist or primary MD to whom results or reports should be sent.
