2004 Authentic Swing Charity Golf Sponsorship Sign-up Form

On July 19, 2004, the ALS Association of Greater Los Angeles will host the 3rd Annual Authentic Swing Charity Golf Tournament!  Last year's event raised over $80,000 for the chapter.  This year's event promises to be the biggest golf fundraiser ever for our chapter!  Thank you all for your support, donations and participation!  Please fill out the following information if you or someone you know would be interested in becoming a sponsor for this even.  To sign up to play in the tournament, click here!
